Post-splenectomy counts

For those who have had a splenectomy I have some questions about where your platelet counts settled at. If the splenectomy put you in remission, what number did your counts finally settle at? Assuming they were very high after surgery and then lowered a bit. Do you see them going up and down much from your new normal number? And how often are you checking counts now?

If you had a splenectomy and it failed, at what count did your doctor consider it a failure? And how long after surgery did that occur?

I'm just wondering how much play there is in counts during remission. Thanks!

  • Diagnosed in 2000, at 59, after being on moderately high doses of NSAIDs for arthritis. Splenectomy and rituxan both failed (2004). Did well on prednisone till summer 2018--then terrible reactions. Promacta since 11-19.

The day after my splenectomy (January of 2004), my platelets were at 99K. They rose up to 10 days after surgery, at which point they were a whopping 684K. At that point, I had been recently weaned off prednisone. Five weeks after surgery, they fell to 26K, but my doctor didn't put me back on prednisone. They went up (as high as 92K), then down. About 11 weeks after surgery, they fell to 10K, and my doc sighed, and I cried, and started on 10 mg pred/day. It was at that point, we considered the splenectomy failed.

The day after my surgery in /06 my counts were in the 200k range. The first week they were decent and then they dropped back down to where they were before. I finally did Rituxan in Sept/Oct of last year and now I'm in the 300's and I'm finally off the prednisone.

One thing to remember is we are all different. Are you thinking about a splenectomy?

Thanks everyone. Cindy, I am almost 6 months post-splenectomy. My counts started out very high and have been dropping slowly, but hoping they will hang out where they are now for a while. Maybe this is just my new normal. Everything I have read seems like the failures show up pretty soon after surgery. I guess there could still be some up and down, but fingers crossed for a very long remission!

I had my spleen out 3-1/2 years ago. My counts briefly shot up to about 600k, and over the period of about a year fell back to 300k, and have been fairly stable there since. I consider this a grand success!

Hope everything works out as well for you.

Thanks Cindy. And thanks mendenmh, that is a great success. Mine started in the high 400s and are now 112. I know it is still a good count, but the drop is somewhat concerning. I just got over a cold, my first illness since surgery, and i know that can affect counts, but I had my count tested before I started feeling sick. Have you had more colds/flu since your surgery than normal?

No, I have not had any increased incidence of anything.

Me neither. I think I've been really sick once.

JTX,
I find that my plts drop even before I feel any symptoms of being sick. I know everyone is different but it's happened to me several times where I've gotten a low count that was abnormal, then started feeling sick afterwards. I think they can drop at the beginning when you don't even know you are sick.

Had my spleen removed Feb 2010 - right after surgery counts were mid 300's. 1 Month after surgery counts mid 200's and have stayed there for 1 YEAR! I have counts done now every 3 months and they stay between 220-250. I have been very healthy and happy!
